Export preparation · 20 Oct 2026

Prepare your old implant inquiries, without patient contact details

Your team keeps names, phone numbers and emails inside your own systems. Elect Loom’s planned Sprint uses broad inquiry categories instead.

Start with the blank Export template

This CSV contains four column headings and no records. Download it to prepare locally; this page does not accept or store files.

The Audit, paid Sprint and upload service are not available yet. This is a preparation resource, not a purchase or upload.

Keep only the fields you need

Work from a copy, never your original records. Keep the original file in your practice’s approved system. Create a separate sheet with these four columns. Do not copy entire rows and hide unwanted columns.

inquiry_date
The inquiry year, such as 2024. Do not include a month, day or timestamp in this preparation template.
treatment
A broad category, such as implant or full-arch. Exclude clinical notes, diagnoses and individual treatment histories.
lead_source
A broad source, such as website or phone. Exclude referral names, tracking links and account identifiers.
last_status
A broad status, such as not booked or no reply. Exclude receptionist notes and message transcripts.

For example, a fictional row could read: 2024, implant, website, not booked. This illustrates formatting, not a real practice or result. Use consistent categories so inquiry groups can be counted without individual histories.

Removing contact details is not enough

Removing names, phone numbers and emails alone does not establish HIPAA de-identification. Do not send protected health information to Elect Loom.

HHS describes two methods: Safe Harbor and Expert Determination. Safe Harbor removes date elements other than year for dates related to individuals. Existing CRM IDs, medical record numbers and other identifying codes can also identify someone. Leave internal IDs out of this template.

Even broad categories may identify someone when combined. Have your privacy officer or qualified adviser review the complete Export before any transfer. This template does not certify de-identification or legal compliance.

Read HHS guidance on de-identification

Check the finished CSV

  1. Remove contact details, addresses, dates of birth, free-text notes and identifying codes. Do not merely rename or hide those columns.
  2. Save only the prepared sheet as a CSV. Reopen that CSV and inspect every column, including trailing columns.
  3. Confirm each row describes an inquiry, not a patient. Keep any permitted connection to original records inside your practice.
  4. Ask your privacy reviewer to confirm the complete file is appropriate to share. Do not email the Export to us.

A ready-to-send Kit is not permission to contact anyone. Your practice must confirm consent and applicable contact rules before sending Messages. Elect Loom prepares copy; your team uses its own tools and records.
